Summer Public Health Research Institute and Videoconference on Minority

Continuing education credits
If you are interested in receiving 1.3 continuing education credits, please:
- Preregister on the Minority Health Project web site; you will be asked to create a four-digit participant code (eg., last 4 digits of home or other phone number.)
- Write your participant code on the attendance sheet distributed by the facilitator at your site.
- Complete the online evaluation form at the end of each day's session. Your evaluation form must contain your participant code and the CE codes that will be announced immediately prior to each presentation.
- You may pay the $5.00 fee by telephoning the UNC SPH Office of Continuing Education at 1-919-843-8858 and providing a credit card.
- You will receive your continuing education certificate after paying the fee and submitting your evaluations.
